Gastrointestinal Foreign Objects in Dogs and Cats
We have seen fishhooks, underwear, tampons, string, tinsel, corn cobs, tube socks, rocks, sewing needles, rubber bands, peach pits, walnuts, toys, rawhide bones, and small mammals to name a few items eaten by family pets.
Signs of intestinal obstruction are anorexia, vomiting, and diarrhea or any combination or the three. These are also signs of many other many other conditions so if your pet is showing any of these you should consult your veterinarian right away. There should be a complete physical exam to begin with, and then lab work and possibly X-rays may be recommended.
X-rays will help take a look inside of the abdomen and chest to look for evidence of an obstruction. If your pet has swallowed something like a rock or other object that shows up well on X-rays it is easy to diagnose an obstruction. If they have swallowed something like string that is not visible on an X-ray an obstruction can be more difficult to find. In that case we look for an intestinal pattern that indicates an obstruction even if we can't see the object. For instance string may bunch the intestine up like an accordion which can be seen on an X-ray, giving the clinician a high index of suspicion that there is an obstruction.
For those obstructions that can't be seen and there is no characteristic intestinal pattern, a barium series may need to be performed. For a barium series, the patient is administered barium usually by a stomach tube and then a series of X-rays are taken. Obstructions are diagnosed by how fast and with what pattern the barium moves through the stomach and intestine.
If an obstruction is diagnosed it may need to be removed by surgery. The surgeon will make an abdominal incision and inspect the entire gastrointestinal tract. After locating the obstruction they will make a decision as to what type of a surgery is indicated. If the object is in the stomach a gastronomy will be performed and the object removed through the stomach incision. If the obstruction is in the small intestine the surgeon will make a decision as to whether and incision into the intestine will suffice or whether a section of the intestine will have to be removed. It is preferable to make the incision into the intestine as complications with an intestinal resection are greater.
After the object is removed or the intestine is resected, and the incision is closed, the abdomen will be flushed to try to remove any stomach or intestinal fluid, debris or bacteria. With any of these surgeries there are possible complications. Peritonitis and leakage from the incisions or the incisions coming open are real possibilities.
The best cure for obstructions is prevention. Watching your pet closely to prevent ingestion of foreign object is the best option. I realize that it is an imperfect world we live in and watching your pets that closely may not always be an option, therefore if you see any signs of illness compatible with foreign object ingestion contact your veterinarian at once.
